Appointments

There are two ways to schedule your MRI appointment at Westbrook Open MRI:

Call us at (708) 836-9000 or

Fill out the Appointment Request Form below.

You will be contacted by our staff within 24 hours to confirm appointment time and details. Stat or urgent appointments must be phoned in. After you receive a confirmation from us, you will need to fax the following information to us at (708) 836-1860:

1. Picture ID
2. Insurance card (both sides)

The day of your appointment, you will need to bring the above, as well as:

1. Doctor's order
2. Email confirmation of appointment

Please be aware that many insurance companies require pre-certification prior to the scan. Please check with yours!
